You can use the Contact Link button to include the customer feedback form as a link among your other review site links.
To add the Contact Link button to your landing page, follow the instructions below:
1) Go to Dashboard > Funnel > Links > Your Review Site Links and click Add a Link.
2) Scroll all the way to the bottom of the list and locate the Other Link Types section. Click Contact Link.
3) Upload a link image or add link text to display on the button. Press Submit when finished.
